The clinical skill of taking and recording patient observations is fundamental to patient care and safety. Healthcare support workers regularly undertake these measurements and need to be equipped with the relevant knowledge and skills to ensure accurate readings. This article provides an overview of pulse, blood pressure, respirations, oxygen saturations and temperature measurements, with the normal and abnormal values given. Both the background and rationale for the use of early warning scoring systems are also provided.
